I haven't been on here in awhile, so here's a quick back story.
We had some growing concerns with our now 19 month old, so we set up our eval from the state. The service coordinator came out to do the intake paperwork, then it was a month later when they came out to do the ST, OT, and DT evaluations. (That was December 12th). At the end when we were making our IFSP, it was decided that we would be receiving an hour of DT, an hour of ST, and they wanted to do a PT eval as well because they suspected some core muscle issues.
January 4th I received a phone call from her, giving me the name of the DT that would be calling to set up our appointments. The DT called later and had an opening for Saturday which was great since both H and I could be here. I asked the coordinator if I should have received anything in the mail, such as the IFSP, and she said she had mailed on December 29th. I just got that envelope yesterday, it wasn't mailed until January 5th, after I asked about it. It's also 10 pages long, but we were only given page 1, then pages 7-10. Would there be some reason we shouldn't receive half of the pages? I already need to email her to get my name spelled correctly (not sure if that would affect anything service-wise). There's also no mention anywhere about a PT eval.
From what the DT says, the coordinator is fairly new still, so maybe she's just getting used to the ropes? Idk.

So you are already getting services less than 1 month after your initial visit? That's on the "quicker" side of normal for Birth to 3.

Over a holiday, I can totally see where a package would have been prepared by a therapist or coordinator and handed off for mailing but not turn up at the USPS for a few days. Especvially if the admin who manages mail took vacation time.

This is a question for the person who mailed it to you. If it were me, I'd google a sample IFSP from your state and see what's on those missing pages. Are they procedural safeguards? Are the results of evals already completed and in your hands? Could they be coming from the DT or SLP rather than the coordinator. I wouldn't assume something sketchy is afoot. I'd ask.

I doubt it, but sheesh- get the names right.

I wouldn't expect the eval to be included in a service plan unless it referenced benchmark evals.

Finally got our IFSP yesterday. They added an extra digit to our address, which must have been what caused the delays. I'm quite astounded by the amount of spelling errors though.
The pages that were missing from the copy I had received were all the scoring pages, as well as the write ups from the therapists that performed the evaluations. She also left me a voicemail the other day regarding a PT eval that she set up, so at least that's being taken care of.

The misspellings are a pet peeve of mine. It's sloppy- as is getting your name and address wrong. That said, sometimes therapists and teachers who suck at writing excel at teaching/therapy.

Exactly, it is really sloppy. The therapist reports were fine, it was the plan prepared by the service coordinator.
