I liked having the flat-fold cloth diapers for burp cloths. I usually sew some cute fabric to the middle and it's an easy and nice homemade gift. Most others have already been mentioned: nail scissors, nosefrieda, muslin blankets, breast pads, water bottle and snacks. Maybe a pacifier clip? I like having a diaper ointment stick to throw in my diaper bag (like the grovia magic stick www.amazon.com/Magic-Stick-Natural-Diaper-Ointment/dp/B002RT8OQE ). Oh and to go along with the lactation cookies (I gave this as a gift to one of my best friends too!) maybe fill a mason jar with all of the dry ingredients and attach a card with the directions to make a batch for herself later on. You could also bring her some freezer meals and a gift certificate for a cleaning service.
All of the ideas mentioned are awesome, so I won't repeat everything, but here are a few more:
-GC for takeout/delivery or make her some freezer meals when it gets closer to her due date. I was so thankful for these after both kids! -DVDs of her favorite shows or a Netflix subscription -Books/magazines -GC for a pedi -Coffee -a giant tub of coconut oil since you can use it for anything -nice water bottle
I'm currently planning a baby boy shower gift and a baby girl sprinkle gift! The baby boy gift is for a first time mom, so I'm gonna put in a teething necklace for mom. I have one that I love. You wear it, they chew it.
Halo Swaddle Sleep sack. White noise machine. My best friend also put together 2 diaper bag for me. She put literally everything you need in there inside.

I got a little briefcase kit of 'little remedies' brand stuff that had gas drops, saline, chest rub and a few other things in it. I loved having it available with my first baby.
I love all the other suggestions you've been given. A couple other suggestions I didn't notice that DD loved was anything that crinkled (either baby paper or she has a crinkle book) and a baby faces book. DD still loves both of those items She also loved her taggie blanket/lovey. Last idea, one of the Boon grass/lawn drying racks for bottles/pacifiers/pump equipment. We have literally used it everyday since she was born, whether BF'ing or FF'ing.

Since it's a friend you want to splurge on, I'll suggest a pj set with nursing tank, pants, and light robe. I got a set for my first shower that has lasted me through 3 kids and is still my favorite thing. People always get so much for the baby I like to do something for the mom especially if it's someone I'm close with.
If she's well-endowed, a bravado nursing tank-- they're pricier than the standard issue target tank but longer, supportive, and hold more pp belly in.

Thanks, this is a great suggestion! Hadn't even occurred to me that a nice set of PJs would be good, but I lived in them for so long after my baby was born I definitely think she'd appreciate it.
I agree with this in addition to the baby items mentioned. My mother gave me a beautiful set of pj with a nursing tank, robe, and shearling slippers that were heaven while I was in the hospital. She also gave me cashmere socks that instantly made me feel pampered and cozy that LO couldn't ruin by spitting up on.
