It doesn't get super cold here so I only have to add a little jacket and hat. I don't like being cold so I don't stay out for long, but for instance, we did take DD out to see Christmas decorations when she was 2 months old.
One thing though that some people may not know (I don't want to assume anyone does or doesn't know) is that in the car seat, baby shouldn't be super bundled up so that the straps are looser.

Totally salking this fir answers too. I was told babies should wear one layer more then what you wear. But I really don't know if that's old or new rules.

Guess it depends how long you plan to be outside for. I don't plan to bundle him up in anything special because he's going to go from the car (parked in a garage) to a 2 minute transfer inside daycare, and again back home. I have absolutely no plans of going to a football game/ festival/ ice fishing/ etc. where he would be outside for longer than 10 minutes. I always keep extra items in the car in case of emergency where I'd have to wait for someone to come get us.
We have the heat on already it's at 67 in my house. I've been having him sleep in fleece blanket sleepers with a short sleeve onesie underneath and he's plenty warm...he has sweaty feet in the morning lol

I don't have exact answers, but I just always touch them (upper back, chest) to see if they feel warm or cold. My hands are generally cold, so if he feels warm I assume he's good.
This morning I took ds1 to an event at a farm. It was about 45 out. I had ds2 in a onesie/pants/socks with a fleece sleeper over it. Fleece hat and then the little no-scratch mittens. I wore him in the ergo, so he had all my heat. He felt warm and wad asleep the whole time.

And a friendly carseat safety reminder: no bulky clothes or coats in carseats!
We dress in thinner layers and a shower-cap style cover over the seat, or blankets. I prefer the cover over blankets because he can't pull it up over his face as easily (once a little older).
In the car seat, never anything thicker than a lightweight fleece. Feel free to pile blankets on over top, but you want very little fabric/fluff between the baby and her straps.
I follow the "one layer more than me" rule. So right now we walk to school with the kids and I in jeans , a long sleeved shirt and a sweater or light jacket. DD wears a short sleeved onesie under a footed sleeper and a blanket wrapped around her. I also put a hat and mittens on my kids at anything below 50 bc they are delicate little mid atlantic babies

I used to keep a fleece blanket for each kid in the car rather than using a jacket because often times it was easier to just quickly bring them inside rather than fuss with a jacket. I'd then tuck the blanket in my bag or into the ergo.

As far as temperature I don't usually keep my little babies out for longer than building to car under 30 degrees. But hey, in Nordic countries they nap in sub zero temps outside, so...
I will either have DD3 in her Zipadee Zip (thin fleece) or under a fleece blanket in the car seat. I have a thick fleece jacket I sewed a babywearing insert for. If it is very cold (for our relatively wimpy winters) I will put her in fleece bunting in the Beco under the babywearing fleece and wear my winter jacket over to keep me warm. I've done cold winter walks that way.

Since we're talking about keeping baby warm, I have a question. Baby girl tend a to run really warm so I'm always nervous to overheat her. Last night we tried her arms out of the swaddle for the first time. For sleep we have her in a long sleeve onsie, socks, and the halo sleep sac/swaddle (wrapped around torso with arms out). Well her poor little arms were icicles just now when she woke up. The rest of her body feels ok so I'm unsure what to do about her arms/hands. I ended up putting her arms in the swaddle and I'm about to put her back down. How do I keep her arms/hands warm?!

So, we're city folk and the baby will be back and forth to daycare in a stroller everyday - summer, winter, etc. It's about a 15-20 min walk.
It's 50s now so she's in a long sleeve onesie, pants, socks, fleece jacket, hat, and muslin blanket.
As it gets colder, I'll add a full body, warm lined body suit that I'll put over her clothes and use a warmer blanket. We also have a weather cover for the stroller to cut down on wind and (obviously) precipitation.
And we have one of those warm-lined pouch things for a carseat that zips up.
I think that some days I may also just carry her under my winter coat. Body heat is great in the cold!
It's still hitting 80°F daily here so I have no idea if we will even need cold weather clothes this year. I'm a fan of fleece zip up "bags" for stroller walks and fleece pjs for car seat rides. If it's really cold, I'd do socks, cotton sleep n play, then a fleece PJ on top.

I never found a good hat for either DD or now DS bc of small heads. As a result DD couldn't see most of the time (hat would fall into her eyes haha). She ended wearing 0-6 month hats until well past she was a year old.

Costco has nice one-piece hooded fleece outfits right now for something ridiculous like $6, for anyone in cold climates. They would be perfect for car seats on cold days

Honestly, I think mittens are a lost cause... kinda like socks. A never ending battle of keeping them on and keeping track of them. My kid is small so I have to roll up sleeves. I don't to keep them over her hands or I put them under a blanket. But, in the end, she does as she pleases.
Do you still have their hospital hats? Can you put those on and then another hat over it when it's really cold?

bocaburger, I have the opposite problem. Had like 8 million hats that were/are too small for his noggin! Seriously, some said 9-12 months and I already gave them away...I swear they must have been labeled wrong. Wish I could have sent them to you lol
dc2london, well I live in MA and I act like DC people during snow. There's an inch? Work from home. They are forecasting any at all for the next few hours? Work from home. I once had to drive in a blizzard when I was 18 to make it to my college final exam and it resulted in me spinning around 3 times on the interstate. So now I do not leave the house for anything. lol
