I hope surgery goes off without a hitch. I used to help with the bariatric program at my last job and saw some great results. Just follow all the doctor's instructions and you'll do great!
One of my besties did the gastric sleeve in February. She went through a lot of the emotions you're talking about. She said it was rough for a bit after. But now, 2+ months later, she's down 40 pounds already and said it was the best choice she Ever made.
You will do great. Like trtlcrzy said, I think the most important thing is to follow the dr orders

I'm proud of you for making the deduction to put your health first. Never be ashamed of that. It sounds like you've done a lot of research and taken all the right steps. I hope the recovery isn't too rough.
I agree with the others, what matters is that you're taking the steps to get the steps to get healthier. Because I think we all have a few "coulda/woulda/shoulda's" from our youth. Wishing you a speedy recover!
I had no idea they have you jump through so many hoops beforehand. How closely do they monitor you afterwards?

The hoops are usually mostly insurance requirements. Every insurance is different with Medicare having the least amount of "hoops".
