I've come to the conclusion that kids are gross. DD has done it twice in the last week. We had to move her up to 18 month pants, but a few of them are too big in the waist and she pulls them off and her diaper. We don't cloth diaper, so I'm not sure if the type of diaper has anything to do with it. I wish I knew how to stop it, but I don't. I just give her a bath immediately, and try as best as I can to disinfect her bedroom. I hope you find something that works.

We CD as well. Thankfully DD wants nothing to do with her poop. There have been a few times her CD leaked and she freaks out as soon as the poop hits her leg. My FIL had twins and one of his famous stories he tells is when my SIL's were little and pulled off their diaper and spread poop all over the walls. I hope this never happens to us. Fingers crossed!
I afraid to even answer this for fear I'll jinx myself. No he hasn't done it, yet. We use cloth too. He will usually come up to us and grab his diaper to let us know that he's going and we change him. If he's playing outside he won't come tell us, but he's too busy doing other things to play in it.
