I just wouldn't even know where to start. DD is my first and I've never formula fed. I give her tap water in sippy cups so I am assuming I can just mix formula with tap water at this point. How much do I give her? She normally drinks a 5oz bottle of breastmilk. Lately at day care she has been taking maybe 3 oz bottles.
Yes, you can do tap water as long as you aren't on well water. I use the filtered water from my fridge because it makes me feel better.
I gave the same number of oz per bottle - would just replace one bottle with formula. Some people give the BM first and a couple of oz of formula after so that no BM goes to waste.
They say babies take more formula than BM- but I find that it didn't really change because he was used to what he was used to. Especially as at this age, they will be taking less liquids and more solids over the next few months.

You may have to start with bottles that are half or 1/4 formula (mixed with water) and the rest BM. I kept the bottles the same number of ounces. We just use tap water.
This is a good point. Some babies don't like formula. I didn't have that problem - so I would try straight first. Just make sure it's warm enough. If LO is used to body temperature liquids, the formula needs to be warm. It can seem like he's rejecting the taste but it may just be temp.
