I'm really pissed and freaked out now

I take large doses of fat soluble vitamins -- specifically A and D. I had stopped taking them when I got pregnant until I heard from my doctor what to do. His nurse called me 2 weeks ago and said that the doctor said for me to take them at my usual doses. I repeated back to her "The A and D?" she said yes.
I went to my first OB appointment and he was going over my vitamins and meds again and he seemed surprised at what I was taking, and said "You aren't supposed to be taking the A..." then he showed me in my chart where it was noted that he said to NOT take the vitamin A.
So. I've been taking dangerously high doses of vitamin A for 2 weeks because his freaking nurse can't read.
I don't even have word for how I feel right now.
