I miscarried naturally yesterday. My RE confirmed today. This is my 4th loss.
RE is convinced we are missing something, since 2 fetuses we had tested from previous pregnancies were normal. We've already run an RPL panel in the past that showed no issue so I will be repeating the bloodwork next month to see if anything has changed.
My options, barring a different bloodwork outcome, are to try again and say a prayer or gestational carrier.

I'm so so sorry, lots of (((hugs))). Depending on where you are, it may be worth it to pursue seeing a RPL specialist. There's a list of them in that Google doc on the 3T RPL check-in, as well as some other helpful info on testing etc. Be gentle with yourself.

oh crap I'm so sorry to hear this. Sending you so many hugs (and so much wine). I hope you get answers. I'm so sorry you're going through this again. Holding you in the light
Since you've seen an RE in the past I'm assuming you've had an HSG or some form of uterine imaging to rule out any structural issues?
Yep. The HSG came back totally normal during my original testing. Which was great but kind of frustrating.
And re: RPL specialist, I honestly don't have it in me to seek another doctor. My current RE plans on providing antibiotics for a kind of uterine infection that is easier to treat than test for (forget the name...fuck) so we will be doing that, plus progesterone and baby asprin plus weekly ultrasounds in a future pregnancy.
I am willing to try one more pregnancy before calling it quits on carrying a biological child. It'll be our hail mary, and if it doesn't work out, we will pursue a surrogate or another adoption. At a certain point, you gotta stop slamming your head against a brick wall, KWIM? I appreciate everyone's support. This community is unrivaled in its knowledge and caring.
Sending SO many hugs. I have been watching for updates. Definitely redo your testing. I had to do that, but I also switched REs because I felt like the 1st RE I saw dropped the ball. RE #2 was concerned about y positive MTHFR result and recommended BA and lovenox. Whether that made the difference, no idea, but I'm glad he was more proactive.

Bring on the motherfucking wine.
Fuck. I'm sorry. Sounds like we are in the same boat test wise. I'm so sorry.

I would ask your RE to perform a hysteroscopy so they can see directly into your uterus - it is much better than a HSG and can pick up things a HSG can miss. Besides the APS stuff, make sure they are checking thyroid, thyroid antibodies, blood sugar and Vitamin D. I would request a clotting panel since some of your losses were later into first tri if you haven't had one done already. I was also tested for Celiac disease since it is much more common in those who experience infertility or miscarriages, and can be present without major symptoms. I'm guessing your RE is referring to endometritis, I was also put on a Z-pack to treat a possible infection after my second loss.
I'm pretty much unexplained, so I understand your frustration. My new RE suspects lining issues as the cause of my losses, so I am starting a Tamoxifen medicated cycle next month since it is supposed to help build up the lining. Progesterone wasn't enough for me. If that doesn't work we will move to injects/IVF since it produces better quality eggs/lining than the oral meds.
Most REs won't investigate further than recommending BA and progesterone unless they have interest in RPL. Make sure you start the progesterone vaginally (400 mg) at 3 DPO for it be effective. I'm also on a bunch of supplements that are supposed to help egg quality, feel free to PM me if you want to know about them. Good luck with the rest of your testing, I hope you get some answers.
I'm going to second the advice to seek a second opinion - especially before you jump to surrogate. Your RE is missing something and the game plan is very . . . concerning to me after 4 losses with 2 genetically normal pg'sverified.
Reading what you posted and from what I can remember of your loss history, circulation and implantation issues should be explored. (This lead to an endo scratch as well as neupogen wash pre transfer for me).
Have you had a full Counsyl genetic screen done? This could show a few different things you may have or be a carrier of (that's how we found my MTHFR mutation) which could effect the protocol they give you.
Also, when was the last time your RE checked your TSH? Was it over 2.5? If it was and your RE not address this, move on to a different RE!
More importantly, step back and love on your family and let them love on you. Wait, heal, and then think about what your next steps are.

i'm not going in for testing for another 4-6 weeks, when i get closer i will be making a list of everything i need to follow up on to truly make sure i know everything that is being tested. i will definitely be repeating a clotting panel (i had it run originally) since autoimmune disorders run rampant in my family history.
I don't think my RE was referring to endometriosis, since i remember not recognizing the name of the infection when he told me what it was, and obviously i've definitely heard of endometriosis. not that it couldn't be, but... i don't think it is. who knows. my retention wasn't great yesterday.
My RE is fairly adamant that we are missing something in the bloodwork, not necessarily that it's a progesterone deficiency (which was what i believed, and i'm certainly eating my confidence.) My RE thinks that my low progesterone this round was because the pregnancy was never viable. i asked him if progesterone while TTC would help, and he said no, since I don't have an issue with luteal phases/i am able to get pregnant very easily/all my past pregnancies were otherwise healthy. He is generally risk adverse which was frustrating to me in the past, but he really stepped up yesterday and is willing to test anything and everything "but the kitchen sink" to see this through. Although he doesn't specialize in RPL only, our conversations yesterday proved to me he is definitely competent to the point where it would be lost time and energy to switch to yet another doctor, and after some research his practice works in tandem with the RPL clinic i would switch to anyway. I admit that a lot of my adamancy is due in part to piss poor experiences in the multiple times i have switched doctors in the past, but i do truly think he will get to the bottom of this, one way or the other.
i am just so upset and annoyed. despite this being my 4th loss, i feel like i can't even "count" it towards the root of the problem since it's considered a chemical pregnancy- the exact opposite in every way from my previous losses. Where as we know the 2 of the 3, and likely the third, were all normal, we know that this was not... so i'm trying to sort through my feelings of bad luck and worry that something otherwise asymptomatic and serious is wrong.
You've definitely given me a good head start though, in knowing what to ask for and having clarity about what needs to be looked into, and i appreciate it. a big mistake last time was that i never asked for a copy of my results- this time i will be analyzing that shit as soon as the results come back.
