The time and a half depends on your state. Some states pay overtime when hours worked for the day exceed 8 and some pay when hours for the week exceed 40. I'm in a by the week state.
Again state rules may differ but I don't think you are obligated to do vacation or sick time for part time personnel. Others should weigh in on this one bc I'm not 100%
You would probably reimburse mileage.
I think there are websites/resources that help with how to do the reporting etc. there was a thread about this on working parents a few months ago. They would be a great resource for this question. Post on there too

How do people do this in practice? Do they have a babysitter who comes for an hour a day? I've been pondering this question and can't figure out how it would be remotely feasible.
That is a very good question. Maybe during nap time or something? When I wrote it I didn't think about reality.
I would plan on paying her for all the hours she is at your house. Yes, she may get some downtime during naps, but she's not really off the clock as she's basically on call the whole time.
For a part-time nanny, I wouldn't really expect you to provide paid vacation or sick days. Basically, she gets paid when she works. I would pay her if you canceled a scheduled work day with short notice. If you know you won't need her a certain week (e.g. when you are going on vacation) and let her know that well in advance, I don't think you have to pay her for her normal days, but it would be nice to pay her something (maybe 50%).
My sisters nanny works 6-4 3-4 days/ week and doesn't get paid time off or scheduled breaks. She's on all day. They use care.com to do the taxes for them.
