Ttma behavior

in the fall we started ds1 on clonidine. In an effort to improve his sleep to reduce daytime impulsiveness.
he has some severe ADHD going on lately. But it's random. He will have very good days and then terrible days like today where he is pulling hair, clothes, throwing up from shoving food in too fast, etc.
There has been no pattern or antecedent to the bad days
our neuro wasn't comfortable prescribing beyond the clonidine.
where do I go from here? He has some severe ID, SPD, and ASD tendencies. When you add all that onto the ADHD stuff it's a disaster.
im not sure if he would get an actual asd diagnosis, his issues are because of his diagnosis. He has zero speech or real communication, doesn't point, only just started clapping a bit last year. Yet, he has great eye contact with kids and adults, wants to be around and watching kids and what they're doing. He has some sensory defensiveness around loud noises but that is it. He eats everything, etc.
but when his ADHD flares up he can't even make eye contact for more than a split second, he can't walk in a straight line without falling or tripping over his own feet. He can't sit next to a anyone without pulling hair or their clothing.
advice? What dr to see? Etc?
