I'm going to suggest a format for this, feel free to tell me this sucks but it made sense in my head. For the most part I'd think the questions relate to the previous week, like a recap.
Eats: (a place to put any eating plans, meal plans, recipes, I dunno)
Sweats: (recap exercise from previous week?)
This week's goals:
Long term goals:
Highs/Lows: (Ex: I lost 5lbs! I ate Cadbury mini eggs for breakfast. Crap like that)
QOTW: What is your motivation for getting healthy?
Sweats: Last week I went to Crossfit 3 times. I will go at least twice this week, and I'm planning some exercise for this weekend (walking/hiking and volleyball).
This week's goals: I want to see the scale go in the decreasing direction.
Long Term Goals: I would like to lose 75 lbs and become fit and toned. For Crossfit, I want to be able to do a strict pull-up.
Highs/Lows: I did a bit better at drinking water yesterday (for me - still not great). I had cake and ice cream last night for a snack At least it's gone now though.
QOTW: My motivations are doing something good for myself and TTC.
Saw RE 1/11/17 HSG x 2 - 2nd revealed both tubes open and arcuate uterus Cycle #1-2 Femara 5mg + TI = Cancelled - poor response Cycle #3 - Femara 7.5mg and Dex 0.5mg - another poor response - waiting to see when I O

Eats: (a place to put any eating plans, meal plans, recipes, I dunno) I am currently doing nutrisystem.
Sweats: (recap exercise from previous week?) I swam 3 nights and rode my bike once which is my goal right now.
This week's goals: Same as last week. Stick to the food and exercise.
Long term goals: Lose 24 more lbs.
Highs/Lows: (Ex: I lost 5lbs! I ate Cadbury mini eggs for breakfast. Crap like that) I lost 2 lbs last week so right where I want to be! I have some sort of stomach bug or something so I'm going to stop eating my regular food because it seems to be aggravating it. I don't really want to because I'm so bad at portion control and I'm on a roll! I guess it's a positive thing that I don't really want to go off the diet!
QOTW: What is your motivation for getting healthy? I am hoping losing some weight will relieve some of my back problems so that's my motivation.

I like this!
Eats: VEGETABLES
Sweats: I'm being terrible. The weather was beautiful this weekend and I still managed to make excuses to not get out and jog.
This week's goals: I will job three times - today, Wednesday (even if it's raining!), and either Sat or Sun.
Long term goals: For a hiking/backpacking trip in June I want to be able to run 3 miles at a 10min/mile pace, and hike 10 miles with a 30lb pack or a baby strapped to me.
Highs/Lows: Today is marathon Monday in Boston. This means that the city is SWARMED with disgustingly fit people. Meanwhile my ass is sitting on a chair in my office. I also had some nice juicy roast beef for Easter dinner last night (it was worth it).
QOTW: What is your motivation for getting healthy? I feel better and sleep better when I'm physically fit. Also, I was in really good shape before my last pregnancy and was able to keep exercising throughout. I think this contributed to the fact that I had a pretty easy pregnancy, so if I get pregnant again I'd really like to have a repeat experience like that. Long term? I'm getting pretty ancient so I want to still be alive when my son goes to college/gets married/has kids/does whatever other big life things his heart desires.
Eats: (a place to put any eating plans, meal plans, recipes, I dunno) I'm not really on an eating plan, more just don't eat all the bad snacks all the time. And take healthier portion sizes of what I am eating.
Sweats: (recap exercise from previous week?) I did the BBM 6.0 90 day challenge with some ladies on Parenting and it's last day was Saturday! Super proud of myself for completing it. I've also been doing a monthly challenge with a local group since January and have been keeping up with it.
This week's goals: Start a new BBM challenge today! Keep up with my local challenge.
Long term goals: Maintain the activity level/healthier lifestyle. I'd like to lose more weight but I'm super excited about how much stronger I am since beginning these challenges.
Highs/Lows: (Ex: I lost 5lbs! I ate Cadbury mini eggs for breakfast. Crap like that) As stated above, just super excited I finished my first BBM challenge! So by finishing that this weekend, CD 1 and family Easter supper all falling on the same day..well I gave myself a free pass on all things food and candy this weekend haha.
QOTW: What is your motivation for getting healthy? When I first started I just wanted to lose weight. But it's become so much more than that already. Feeling less guilty about how I'm treating myself. Putting some routine into my SAHM life. Watching my kids learn and get excited about it, especially my oldest who now cheers me on. ETA knowing we wanted to start TTC this year was the first big push to get me going again too!

Eats: I made mason jar salads last week but then I went on antibiotics for my leg and it made me very sick and is suppressing my appetite. The salads are now going bad while I eat apple sauce and yogurt.
Sweats: none, I'm not supposed to walk much.
This week's goals: survive this antibiotic
Long term goals: lose lots of pounds.
Highs/Lows: Last I checked I lost 3lbs in as many days. But it's definitely not how I want to lose.
QOTW: What is your motivation for getting healthy? To get healthy. These past two years have been horrible.
Eats: I need to meal plan for the week and I'm going to start eating the same thing every day for breakfast/lunch and snacks until I can get a hold of portions, etc. Otherwise, I'm all over the place thinking I'm eating good, but in reality, I'm not!
Sweats: Last week I got in a little over 10 miles of running (1 was a stroller run) and one weight workout
This week's goals: Run 3x, try to get in 2 weight workouts
Long term goals: I need to lose 10 lbs to be pre DS weight, 20 lbs to goal weight
Highs/Lows: I found a free boot camp in my area that I can attend! I plan on starting next week.
QOTW: What is your motivation for getting healthy? I want to feel comfortable in my clothes and I want to be healthy for my family.
Eats: I ate terribly last week - I was swamped with work and didn't cook, plus Easter candy. Need to get back on meal planning this week!
Sweats: Walked when I could at lunch, and played some tennis with DH this weekend
This week's goals: Meal plan and cook healthy dinners; walk every day at lunch
Long term goals: Lose another 5-10 pounds; get in better shape overall
Highs/Lows: High: Played tennis for the first time in many years and wasn't as terrible as I expected, Low: I ate all the Easter candy
QOTW: What is your motivation for getting healthy? Fit in my clothes better; get to a better weight for IVF. Also, I carry my weight in my stomach, and nothing is a knife to the heart quite like people congratulating you on your non-existent pregnancy when you're on year three of TTC.
Eats: I think I consumed only sugar last week. Fail.
Sweats: I've got the exercise part down. Last week I did 3 runs, 4 beach body videos (21 day fix and Core de Force), and a few walks.
This week's goals: No sweets, and minimal processed foods. Also to lose 2 pounds.
Long term goals: lose 12 pounds and be able to do 20 regular push-ups
Highs/Lows: I increased my weights and didn't die, but also ate all the junk food.
QOTW: What is your motivation for getting healthy? I feel so much better when I eat healthy and exercise, which helps me be a better mom, a better person in general. And I want to look good in a swimsuit. Meh.

Eats: I need to start eating breakfast again. I've been skipping it all week, for no other reason than I'm tired and I don't want to get up that early. Coffee does not = breakfast M!!
Sweats: Ugh, nothing but work.
This week's goals: Get out for a walk at least once. I'm working all weekend and we leave on Monday for our trip, but tomorrow looks like it'll be nice enough to get out. Might try Friday too.
Long term goals: Lose another 30-40 lbs to get to my goal weight
Highs/Lows: Eating sooo much crap, and way too much of it too t.t I think H was feeling pity for me when I was down with my vertigo because he was buying me things he doesn't normally (donuts) AND he took me out for buffet at the casino. I love him for wanting to take care of me, but c'mon now
QOTW: What is your motivation for getting healthy? Being healthy makes me feel good, and eating the right things has stopped my stomach aches for the most part. Plus TTC. I don't want to be an overweight momma!
Eats: (a place to put any eating plans, meal plans, recipes, I dunno) FI and I have been doing South Beach Diet for the past month. At last check, I was down 11lbs and he was down 7 or so.
Sweats: (recap exercise from previous week?) None. I did just get new swimsuits though this week so I can start swimming again now that it's hot as hell outside.
This week's goals: Avoid fast food I had a few slip ups last week but I've been on the right track this week.
Long term goals: Get to a healthy weight
Highs/Lows: Easter was a shit show diet-wise. Also, FI's birthday was yesterday and we indulged in some cake.
QOTW: What is your motivation for getting healthy? Managing my arthritis, having a healthy pregnancy
ajetter, 20 pushups! I have never been able to do one. I don't know what it is about them. I lift my 40lb son a million times a day, you would think I could do at least one. I can go like 1/3 of the way down. It is ridiculous.

I'm that way with pull ups. At my strongest I was lifting a lot, and only needed about 10lbs of assistance on a machine, but for the life of me could not do a single freaking pull up without assistance. I can do about 10 push-ups now. I started the beach body videos in January and I could only do one if I was lucky!
