Extra rinses couldn't hurt I imagine. When we had Tide build up I had to use RLR to really get it out so if you want to be extra sure then I would do that.
Sorry if it has been asked but I assume you confirmed the rash isn't yeast.
Have you tried switching diaper creams? When M's rashes get bad our usual just won't cut it.

It started out as yeast, I think, and we treated for it. After a couple days of the yeast treatment the rash was almost gone for a day, then it came back and looks completely different and now I cannot get it to go away. It does get better at times, and it's not blistering or bleeding or anything and it doesn't hurt her.
I've tried all the creams, both cd safe and not. And coconut oil with tea tree. And breast milk. And no creams. And cutting out dairy. And air time. And baking soda baths. And a couple different brands of disposable diapers. Everything! It's ridiculous.
I think I do have an RLR packet around here somewhere, I'll take a look.
Yikes! That sounds terrible and frustrating! I hope the detergent switch works! If not maybe some different anti fungals-I know yeast can be horribly persistant.

I see you cut out dairy, but what about citrus? Do you feed her any baby food yet?
Back to laundry, I heard about some washers recycling water on the last rinse. Do you know if yours does that? Maybe the diapers are getting clean, but not totally clean.

Which disposable? And are you using them right this minute? Or are you back to cloth?
Huggies gave my baby a red rash. It was always there. Nothing would clear it up. We use the target brand diapers now and don't have issues.
Also, do you use cloth or disposable wipes? We used to use Huggies, but now we use the Kirkland wipes.
Wow, i just realized how much of a fail Huggies turned out to be for us.
Anyway, I know you said you tried all the creams, but if you haven't tried this, I recommend it. Boudreaux's Butt Paste was a life saver. Not cd safe though.
We went through months of terrible diaper rash, (I almost gave up cd) so my heart goes out to you.
@milano, total anecdote b/c obvs every kid is different, but when I cut dairy, I wasn't thinking about soy and she had a very persistent rash (contact dermatitis, basically) for forever. until we got her allergy tested and soy came back positive. I cut it from my diet and within a couple days, her skin improved dramatically. Yes, it's hidden everywhere. But it may be worth a shot over the weekend to see if anything improves before her monday appt. Just eat all fresh foods over the weekend (i.e. - no prepackaged stuff). soy and soy lecithin is everywhere. I hate soy with a passion now LOL
Eczema? Have you tried the eczema stuff? Like bleach bath, waxalene, etc?

Most definitely. My eczema is just on my hands and feet. Eczema has a trigger (usually and allergy), so soy/dairy could still be an issue. Or the trigger could be environmental, uncontrollable or completely unknown (so the treatment would be on the dry/itchy skin rather than trying to control the trigger).
I was thinking about this last night and her being fine in the morning and getting worse throughout the day (regardless of diaper type) says to me that the problem could be with her poop (since she doesn't poop for the longest stretch at night). Which could be an allergy to something. But you could also try (and maybe you already have) slathering her with something thick enough to keep the poop off her butt. I'm not sure what that would be. Vaseline? Drywall spackle?

LO had super sensitive skin. He wore disposables for the first 4 months and we had to lather him with butt paste every single diaper change to keep his diaper rash controlled. That is one of the reasons we switched to cloth and even then we had to do baking soda baths and lots of air time. His wasn't nearly as bad as what you are describing, but one thing I did to help soothe was to add a little baking soda to my wipe water.
That's how I feel about methylisothiazolinone (MI), methylchloroisothiazolinone (MCI), and a few other variants. They're in soooo much stuff and not always included in the labels - example sometimes they're used in the fragrances but manufacturers don't have to disclose the chemicals used in fragrances.
