I guess technically Leo is a preemie. He was born at 36w3d with severe iugr weighing 3lbs 8oz on April 28. We are still in the nicu 11 days later doing the feeder/grow.
He's taking all his bottles and slowly gaining but it looks like he'll be hanging out for awhile just getting bigger.
I have a 15 month old at home (which is 2 hours away) I have no idea what's going to happen once we go home, and to make matters worse I was an inpatient for 6 weeks prior to delivering.
Any advice on how to make the transition smoother? Is there anything I should REALLY be on alert for with my small guy preemie / size wise? Are preemie diapers really worth it?

I have 2u2 with a preemie - my best advice is to take it one day at a time. The first couple months were difficult for us, but we got into a good routine and then it got better. Congrats!
Congratulations on Baby Leo! I absolutely love the name, BTW. Yes, 2u2 is hard. I did it with two preemies - a 32 weeker and a 36 weeker 18 months apart. It is rough at first, I won't lie. Accept all the help you can get. Give your 15 month old as much attention as you can, but don't feel guilty when you have to tend to the baby. He or she will adapt.
As far as the preemie questions, jnetx said it perfectly! The NICU care team will help you. I would assume you'll be in to see your pedi for frequent weight checks. They will give you plenty of guidance. As for preemie diapers, DS1 (32 weeker) came home at 5 lbs. 4 oz. and we used preemie diapers for a couple of weeks. DS2 (36w) came home at 5 lbs. 2oz. and we never bothered with the preemie diapers. Good luck with the transition! You will get through it!

My NICU nurses switched the boys to Newborn diapers before they came home. My guys were born at 2.8 and 3.2 and when they left NICU they had just his 6 & 7lbs.
