RH factor and rhogam shot for miscarriage?

This is my first pregnancy. I am 8w3d and am most likely headed towards a miscarriage as the baby has only grown 1.24mm (2days) in a 7 day period of time (we were measured with t/v ultrasounds and its measuring over a week behind). I am O- and my husband is AB+. My RE has said nothing yet about coming in for a Rhogam shot if I start to spot or miscarry. So I have some questions:
1. If you have a negative RH Factor, your partner has a positive RH Factor, and you have a threatened miscarriage or are going through a miscarriage, do you always need the Rhogam shot to protect yourself from being sensitized and developing antibodies? Or does it depend on how far along you are? I've seen some sources say before 12weeks its not needed, and other sources say it should be offered after 6 weeks.
2. If the shot is required, should you get it before you start bleeding? If you get it after you start bleeding, do the people making the recommendations mean within 72hrs after the first time you spot any blood or within 72 hours of passing all of the products of conception?
