Blended Family Issues/Special Needs

SO this was 26 pages of backtracking. I think most of us are still pretty confused as to the details but we are never getting the whole picture so it is hard to give advice.
The picture you painted is not a pretty one. It paints you as weak and ineffectual, both as a partner and as the mother of a child that has some kind of medical issue happening. Everything you have written points to your SO as being a non-involved parent. Your ILs seem terrible, but they also basically raised your stepson because his own father was unable to.
The rub in all of this is that you have a little boy that is suffering. Suffering. And none of the adults in his life can get their heads out of their ass long enough to do something about it. When a 3 year old is not sleeping more than 2 hours a night for any length of time, you take him immediately to the doctor. This constitutes a medical emergency. You guys waited until after he hospitalized 3 children to say 'hey let's take him to the doctor' because mommy in law told you not to. There is no adult in his life that is being responsible here. This is medical neglect. The agency I work in is a foster agency and an agency for persons with developmental disabilities. Kids get pulled from homes for medical neglect.
You may be a decent person, you probably are a decent person. Be this boy's advocate. You have no legal rights, you say. You have a moral obligation to this child, to any child you see that is being mistreated. Because this boy is being mistreated.
