Have you tried hand expressing a bit at the start to relieve some pressure. I did this with DD when my milk first came in and then gave her the expressed bit at the end after she finished (I couldn't bring myself to waste any).

I usually will express some onto a towel first. But even when the flow is slow LO still chokes. I really should be saving the expressed milk, it's a lot!
jgesi I also like the side-lying position for forceful letdown. I just BF LO over a towel and he lets the milk dribble out of his mouth while he's nursing, which helps. I don't hold his head in place after the initial latch. Sometimes the position is more comfortable when you prop your other side up with pillows.
It sounds like you're doing everything right. Sometimes it's just a maturity thing and as LO grows they get much better at handling it. DS1 eventually got to the point where he expected the fast flow and would prefer the faster flow breast.

I whipped coconut oil together with some lanolin. It's pretty much amazing.

I was nervous about putting that in my baby's mouth but the pain was unreal and my ob said it was safe. It must've helped bc after working on that latch we went on to nurse for over a year. You could always ask your ob for help if you need it.

This is Dr. newman's all purpose nipple ointment.
It is a combo that can be an rx and made in a compounding pharmacy, or you can make your own from otc meds.
You only wipe off before nursing if you just put it on and there are gobs on the nip.
It's Bacitracin antibiotic cream (I know some moms who have used Neosporin with pain relief too)
hydrocortisone cream for the swollen tissues,
and lotrimin type yeast cream in case the pain is partially due to thrush/yeast on nips.
combine equal parts and apply to nips after feeds.

Try laid back nursing(you lean backwards and baby on your chest), "natural" nursing (you are flat of back and baby on top), football hold with baby sitting up more, or side lying they can help you to get a deeper latch.
Picture how you eat a big mac, you squish it flat, you hold it by your mouth but a little elevated closer to your nose, you open wide and the top of your head goes back and you tilt your head backwards, bottom lip touches first and you stuff in all the top part of the sandwich that you can kind of pointing it toward the roof of your mouth and then flipping it toward the back of your throat.
Yeah that's kind of how baby needs to latch on if you are in a cradle or cross cradle position. it's called an asymmetrical latch, and bottom lip is closer to the base of the nipple and the top lip gets more areola.
so you hold baby in position but put them so their nose is in line with the nipple a little lower than you think they should be. tickle nose with nipple, as they open wide they will reach upwards and the top of their head needs to be able to go back. If nipple is too low, right in line with the mouth, they reach kind of forward and as they open wide their bottom jaw touches their chest and they can't open as wide. So need to allow some movement of baby's head backward and their butt pulled in tight up against your body, with both shoulders equal distance from you, not one shoulder in closer than the other one.
Alternating cold compresses and warm water soaks speeds healing.
Warm water soaks may be good before a feeding, they increase blood flow to the area which speeds the healing process. The warmth may encourage a let down, so in theory baby doesn't have to suck as hard to get initial milk.
Cool compresses after nursing can help with the pain, and take down some of the swelling and inflammation.
You may find that you want to switch those around sometimes as the cold right before nursing can help numb things.
After nursing if you rub breastmilk into tip of the nipple, dry with a warm hairdryer,
then cover nipple in nipple ointment of some kind (earth mamma nipple butter, or Kerry's herbals miracle salve or APNO). be sure if there is damage that you stick a non stick pad on top so you won't stick to the nursing pad or bra.
then apply cool compresses or a bag of peas in your bra to help with pain and inflammation and swelling. Motrin helps too.
hopefulreturn ours is spitting up quite a bit too and we mentioned it to the pedi. yesterday. The nurse suggested elevating her pack n play a little bit on the side where her head is so that it is more comfortable for her at night. I was worried that she was throwing up everything she was eating but she was weighed yesterday and gained over three ounces since Saturday so something must be sticking!
ETA: I didn't realize that post was from days ago!
2 week apt was today and we are still down an ounce from her birth weight. She seems to eat enough and is satisfied after feeding, but they asked us to come back for a weight check on friday. Fingers crossed she is up a few ounces by then.
Something must be going right for us. Had her 2 week appointment today and she is back to her birthweight! We need to add the vitamin d drops now, so when I remember to pick them up...
So right now I'm almost exclusively pumping (supplementing with a formula bottle in the morning if I run out of breast milk). Because I'm storing the milk in the fridge - and when LO is hungry he is NOT patient - I've gotten lazy with warming the bottle before giving it to him. If he'll drink a cool or even cold bottle is it fine to give it to him, or is there a benefit to heating the milk?

queenb, if he's not picky there is nothing wrong with the breastmilk not being warmed. Formula on the other hand I believe HAS to be heated to a certain temperature, so double check that.

How often does your EBF babe poop? I swear this kid has a poop in EVERY diaper. I change him well over 10x a day sometimes! It's wreaking havoc on his poor hiney too
We have an avg of 12-15 changes a day. It's ridiculous. She poops nonstop.
Yep lots of poop here too! Every diaper change! Totally normal! Be aware though, ebf babies will one day taper off to 1 poop a day or even 1 poop every couple days suddenly. That is also normal. Totally freaked me out with my first. As long as they are still having lots of wet Diapers and aren't showing signs of distress they are Ok. There bodies have just figured out a new poop rhythm.

Yep lots of poop here too! Every diaper change! Totally normal! Be aware though, ebf babies will one day taper off to 1 poop a day or even 1 poop every couple days suddenly. That is also normal. Totally freaked me out with my first. As long as they are still having lots of wet Diapers and aren't showing signs of distress they are Ok. There bodies have just figured out a new poop rhythm.
My mom told me I would on Tuesdays @10am. That was the only time I would poop from 3w-4/5months.

She is gaini g weight and the nurse at her 1 week appt. said she must just be a very efficient eater. But it still makes me wonder. They don't need to see her until her three week appt. but I think I'm going to bring her in at two weeks just to have her weighed. Last night she woke up there times to eat. Once for 4 mins, once for 6 mins, once for 17. Then she went almost five hours after that before she wanted to eat again...

Then she'll sleep for 2-3 hours. Sounds like your baby is pretty happy though, if she isn't screaming for boobies every 10 min then my guess is you do have an efficient eater.

+1. Our longest nursing sessions were 20 min in the hospital while working on his latch. All other sessions are under 10 min, and he only takes one side.

We just had a growth spurt but he was eating for 5-10 minutes and done. Now if I put my boob away he loses his mind.

At first we were having a poopy diaper after reaching feeding, which is about 2 hours. Recently, there have been a few wet diapers without the poop.
zonagirl My guy eats 20 minutes max. My first was a 45 minute eater so I was pleasantly surprised when I realized he was a shorter eater. If she's gaining and seems full, she's probably just a fast eater.
